As a case in point, we recently had a 35-year-old patient who presented at the office with a missing maxillary lateral incisor. The visual examination revealed that there was limited space for restoration, plus the patient had a suprainclusion.

So we began with a cone beam computed tomography (CBCT) scan to validate the bone volume. We then planned the implant in the CS 3D Imaging Software from Carestream Dental, using a virtual tooth to determine if the prosthetic emergence would be compatible with the underlying bone volume. Once the feasibility of the treatment was validated, we acquired digital impressions using the CS 3600, Carestream Dental’s intraoral scanner. The impression file was immediately sent to the dental laboratory to create a wax-up, which was then used to generate a second digital model. This file was then merged with the patient’s CBCT scan – using NobelClinician™ software – to complete implant planning and create and order a surgical guide.

Treatment Plan.
The surgery was performed using a pilot drill guide to position the axis of implants and was then completed free-hand.

Once the digital impression was transferred to the laboratory, the restoration design was validated live through an online video conference before milling took place. Less than two hours later, the patient received the temporary crown.

On an aesthetic zone with limited space between teeth, the prosthetic-driven planning enabled us to accurately determine the implant axis in order to place the screw-retained implant in a more palatal position.

By combining implant planning with guided surgery, we achieved a successful prosthetic outcome. Digital impression complements the digital workflow with the capability to immediately place the temporary restoration – thanks to CAD/CAM design and milling.

The patient came back four months after surgery, at which time an RVG control was taken to evaluate the osteointegration. The RVG showed that the gum was perfectly healed, matching the emergence profile given by the immediate loading produced by CAD/CAM. The final implant position and the emergence profile are going to be registered to produce the new restoration. The new screw-retained crown will be produced in CAD/CAM, and we will reproduce the morphology of the temporary restoration, which served to heal the soft tissue. The ultimate outcome was smiles all around. Definitely from our patient, who had a much more pleasant experience and was given a whole new reason to smile. But my staff and I also have plenty to smile about – our workflow was streamlined by the technology at our fingertips, making for a more efficient, effective and timely restoration job. The bottom line is, we had an expert ecosystem of staff, technology and solutions, all working together to create a great outcome for our patient. At the end of the day, that’s why we do this in the first place. And that’s the story behind this smile.
